As my business has grown, I've introduced a contract to help ensure clarity and mutual understanding for all parties involved. This step allows me to maintain the high standard of classes I strive to provide while supporting the long-term success of the program.

The Provider agrees to offer classes in music as per the schedule and curriculum created by Forest Field.
The Client agrees to a contract term of 1/3/6 month(s), commencing upon payment and ending a week before the last lesson.
The cost for the classes is $60 per hour. This fee covers curriculum as well as basic supplies for the Client. A fee for any books or specialized/requested supplies will be additional to the basic curriculum fee.
Payments must be made in full for the upcoming month for weekly/bi-monthly lessons on or before the 1st day of each month. Payments are non-refundable and must be submitted via cash, check, or Venmo.
Any monthly payments made after the 1st day of each month will be subject to a $20.00 late fee for the first day and an additional $5.00 per day until payment is made in full for the month. Failure to pay within 10 days of the due date may result in automatic termination of the contract, with all outstanding fees immediately due and payable. Consistently late payments by the Client, as determined by the Provider, may result in termination of this contract.
If ending classes prior to the contract term is needed, the Client may terminate this Agreement by providing 30 days written notice. The Provider may terminate this Agreement without notice due to breach of contract, consistent late payments, or other actions that disrupt the Provider's ability to offer services safely and effectively. The contract may also be terminated by the Provider for any reason without notice.
Notice of termination by the Client must be submitted in writing, either by mail or email, and acknowledged by both parties. The Client understands that the Provider implements a zero-tolerance policy for physical violence from the client against the Provider or Provider's pets and the Client's contract will be subject to immediate termination.
By enrolling in lessons at Forest's music studio, you are reserving a dedicated time slot for your instruction. Payment secures this time and is not contingent on class attendance. Therefore, missed lessons on the part of the Client will not be rescheduled or reimbursed unless deemed appropriate by the Provider under exceptional circumstances. The Provider retains sole discretion to determine whether a missed class qualifies for rescheduling or reimbursement. If a class is missed by the Provider, a make-up class will be scheduled at a mutually convenient time.
The Client understands and agrees that the Provider is not liable for any injury, loss, or damage to the Client during the course of the classes, including any accidents that occur on the premises. The Client assumes full responsibility for ensuring the safety of themselves and any children under their care.
The Client shall be financially responsible for any damages to the Provider's property, pets, or person caused by the Client at any time before, during, or after the provision of services. The Client understands and agrees that the Provider is not liable for any direct, indirect, incidental, or consequential damages, including, but not limited to, any lost profits, arising out of participation in the classes.
The Provider agrees that any personal or sensitive information shared by the Client's child during the course of instruction will remain confidential and will not be disclosed to the Client or any other party without the child's consent unless disclosure is required by law. This policy is designed to foster a safe and trusting environment for the Client's child to fully express themselves.
This Agreement constitutes the entire understanding between the parties and supersedes all prior discussions, agreements, or understandings of any kind.
This Agreement shall be governed by and construed in accordance with the laws of the state of Texas.
